In the subspeciality of head and neck, benign and malignant conditions affecting this area are diagnosed and treated.

 

Among the most frequent are:

 

  • Salivary gland stones (sialolithiasis)
  • Mumps
  • Tumours of the head and neck

 
 

 
 

These are calcium stones in the drainage ducts of the major salivary glands (parotid, submandibular, and sublingual).

Symptoms
  • If calculi are small they not cause any symptoms.
  • If calculi are large and obstruct the outflow of saliva to the mouth, they cause inflammation and pain, especially when eating as there is increased saliva secretion from the glands.
  • Occasionally, fever and redness of the area may occur when there is a bacterial infection.

 
 

Diagnosis
  • Medical history and examination (sometimes the stone is felt when the salivary gland is examined).
  • Ultrasound
  • Sialography, a radiological test that visualises the ducts of the major salivary glands. An iodine solution is injected through the saliva outlet holes allowing accurate visualization of the duct pathway on X-ray.

Treatment
  • Conservative management:
    Local heat, massage, saliva secretion stimulation using acids and citrus fruits to try and expel the calculus naturally.
    For a bacterial infection, antibiotic treatment.
  •  

  • Surgical treatment:
    From simple extraction of the calculus to complete removal of the gland.

Mumps is inflammation of the parotid glands, one of the most common causes being the viral infection paramyxovirus.

 
The virus is spread person-to-person through the respiratory tract by inhaling saliva droplets produced by talking, or through direct contact with saliva from an infected person.

 

Symptoms usually appear 7-21 days after exposure to the virus.

 

It occurs at any age but is more common in young adults.

 
 

Symptoms
  • Fever
  • Fatigue and general malaise
  • Pain and swelling of the face, in the parotid region
  • Earache
  • Headache
  • Hearing loss
  • Possible infertility in adolescents
  • Meningitis (in unvaccinated people)

 
 

Diagnosis
  • Medical history and physical examination
  • Blood analysis
  • Saliva culture
  • Parotid gland ultrasound

 
 

Treatment
  • Treatment is symptomatic with anti-inflammatory drugs.
  • The majority of the population has immunity to mumps because of the MMR vaccine (measles, mumps and rubella) given to children, the first dose at the age of 12-15 months and the second dose at the age of 3-6 years.
