After an exhaustive and individualised assessment of each patient, our team will determine the target intraocular pressure and indicate the most appropriate treatment to achieve it, preventing deterioration of the optic nerve and slowing the progression of the disease.
Pharmacological treatment
Hypotensive drugs in the form of eye drops are usually the preferred treatment in most cases. They are used to lower intraocular pressure. Follow-up is very important to check their effectiveness, ensure adherence to treatment, monitor progress and control possible adverse effects.

Minimally Invasive Glaucoma Surgery (MIGS): Xen Implant, iStent Implantation
MIGS techniques are a safe and effective surgical option, with fewer side effects and minimally invasive, which reduces postoperative discomfort and speeds recovery. They keep glaucoma progression under control, thus helping to minimise the impact of the disease on the patient’s vision and quality of life.
MIGS procedures succeed in controlling intraocular pressure with the implantation of micro drainage devices (Xen implant, iStent implant) that facilitate the outflow of aqueous humour.
Non-penetrating deep sclerectomy (NPDS)
The aim of non-penetrating deep sclerectomy (NPDS) is to facilitate the drainage of the aqueous humour in a controlled manner. It is a very precise microsurgical technique that allows the surgeon to remove a portion of the trabeculum after cutting the sclera (the white part of the eye wall). The trabeculum is the structure through which, under normal conditions, the aqueous humour drains. This technique does not alter the intraocular structures, which is why it is called non-penetrating, making it a safer technique and with fewer complications than the classic trabeculectomy, despite its surgical difficulty.
Express implant
This is a fast, safe procedure with a shorter and more bearable postoperative period. It consists of placing a microscopic implant that connects the anterior chamber of the eye with the subconjunctival space, facilitating the outflow of aqueous humour.
During the postoperative period, the use of certain techniques to reduce or avoid excessive scarring, which tends to prevent the outflow of aqueous humour, is not uncommon. Depending on each case, these can be performed with laser or by subconjunctival manipulation in the consultation room or even in the operating theatre (needling).
For the same purpose, certain drugs can be used intraoperatively or during follow-up, usually applied locally at the subconjunctival level, either in the office or in a surgical setting. Your specialist will give you the relevant explanations and details of the best option available for your case.
