Anaesthesiology. Do you truly know the importance of #anesthesia for the success of a surgery?

The anesthesiologist performs and anesthesia treatment that is effective and assures minimal complications. The anesthesiologist is present in the operating room both before and after the procedure.

 

The objective is to maintain homeostasis in the patient, an optimal physiological state and dynamic equilibrium of the body. During the surgical operation, physiological changes are produced, that, thanks to different techniques and advanced monitoring, allow early correction of any uncertainty that may occur.

 

Dr. Daniel Jato, specialist in anesthesiology and the Pain Control Unit at HC Marbella International Hospital resolves our doubts.

 

What are the quality criteria followed for the use of anesthesia?

 

In analgesia – whose role is fundamental to the specialty – it is important to control pain, as much during the procedure as post-procedure. Following diverse studies, we know that the sooner analgesia is began, the greater its effectiveness. This is a guideline that we follow.

 

Postoperative pain is evaluated in any quality guide. There are a variety of therapeutic possibilities and techniques that are adapted to each patient, as well as surgery.

 

Emphasis is placed on the use of locoregional techniques which consist of the injection of a long-lasting local anesthesia, that effectively numbs the nerve and the operated area. This is a accomplished with the help of an ultrasound machine in order to directly localize the nerve and minimize risks. Ultrasound-guided regional anesthesia has been one of the greatest advancements in recent times.

 

In the face of the operation, how is an adequate assessment of the patient made?

 

Patients are assessed in the pre-anesthesia consultation following the anesthesia risk guidelines set by the American Society of Aesthesiologists (ASA). We make a multidisciplinary team of specialists available that, in specific cases, optimize the procedure for each patient before they enter the operating room.

 

Normally, we require some standard tests such a routine analysis, an electrocardiogram, and a chest x-ray. In the cases of patients with complex pathologies, we may require more specific tests in order to increase the safety of the patient during an operation.

 

The physical state of the patient is relevant and for this reason, there is an interview in which the patient is asked about the associated pathology of the disease, medication, toxic habits, etc.

 

Furthermore, patients must follow some guidelines referring to medication, fasting, when expecting to enter the operating room.

 

Why is a multidisciplinary treatment important?

 

The presence of different experts improves the safety of the patient. For example, a pulmonologist, a cardiologist, or a hematologist (that measures the components of the blood) are important fixtures in the pre-anesthesia evaluation.

 

During surgery, are there risks faced by the anesthesiologist?

 

In every anesthesia procedure there is an associated risk compared to the risks associated with other professional activities.

 

For example, there is the same amount of risk as that of when you go out into the street that a pot will fall on your head, as the associated risk of someone with a controlled disease.

 

Obviously, the worse the previous state of the patient, the greater associated risk.

 

What are the main complications during recovery?

 

The patient recovery area is the last step before being moved to the medical ward. Here the patient is kept completely monitored, adduction is completed, and the analgesia state is controlled, assuring that the patients conditions before they are moved to the medical ward. This last step is variable in time and depends on the surgery.

 

At HC Marbella we count on the Major Outpatient Surgery unit in which the patient is immediately discharged if they complete certain safety criteria. An early discharge requires specialists experienced in anesthesia techniques. As we have already said, pain is a quality criterion (also) within the area of recovery. The different techniques of analgesia and the existence of intelligent syringes help to control the patient, making them feel more comfortable.
