Up to 40% of women suffer from pelvic floor disorders. They tend to develop with age, during or after pregnancy, following pelvic surgery or after certain medical conditions.

 

Urogynaecological, or pelvic floor disorders, include:

 

  • Prolapse of the pelvic organs (bladder, uterus, vagina, rectum)

Treatment

The choice of treatment for pelvic floor disorders will be determined in medical consultation on the basis of the type of urogynaecological disorder, the age, medical history and general state of health of the patient, as well as the patient’s wishes, expectations and preferences. Treatment options include:

 

  • Bladder retraining
  • Physiotherapy for pelvic muscle rehabilitation
  • Pharmacological treatment
  • Pessary
  • Outpatient consultation procedures: injections with urethral bulking agents (Bulkamid) or CO2 laser treatment
  • Surgery: slings, surgical repair of pelvic organ prolapse, mesh, hysterectomy, colpocleisis (closure of the vagina).

Urinary incontinence is the involuntary loss of urine which leads to social and hygiene problems. It occurs in healthy people as well as in association with other diseases and can have several causes.

 

Incontinence

 

It is calculated that between 30-40% of middle-aged women have some type of incontinence and this percentage increases with age.

 

It is important to differentiate between the types of incontinence and to make an accurate diagnosis as treatments are very different; the following types will be discussed:

 

  1. Stress incontinence (SI): This is the involuntary loss of urine associated with physical effort causing an increase in abdominal pressure (such as coughing, laughing, running or walking).
  2. Urge incontinence (overactive bladder) appears when the bladder muscle starts to contract and expel urine ahead of time, even if the bladder is still not full.
  3. Mixed urinary incontinence.
    It is calculated that 30% of women with incontinence suffer from the 2 types previously mentioned.
  4. Continuous Incontinence.
    This is the involuntary and continuous leakage of urine. This can be due to a vesico-vaginal fistula (communication between the bladder and the vagina)  and usually presents after gynaecological surgery such as a hysterectomy.
  5. Other types of incontinence.

 

In all cases there are measures which can lessen the problem, depending on the type of incontinence and each patient’s circumstances, one or more will be selected.

One of the first lines of treatment for stress urinary incontinence is injections with urethral bulking agents, specifically Bulkamid.

 

Bulkamid is a polyacrylamide hydrogel that is currently being used in breast surgery and has been demonstrated to be non-migrating and non-biodegradable even after ten years of monitoring.

 

The Bulkamid procedure involves 3-4 injections of gel into the wall of the urethra, which helps prevent urine from leaking out of the bladder during daily activities.

 

Many women choose Bulkamid over invasive surgery because the procedure is minimally invasive, can be performed under local anaesthesia, usually takes 10-15 minutes and requires a short recovery time. The complication rate is very low and does not exclude you from benefiting from other treatment options in the future.
